Schedule: MondayFriday, 8:00 AM 5:00 PM | No Weekends, Holidays, or On-Call

Join a mission-driven home health team as a full-time Occupational Therapist in Williamsport, PA . In this role, you'll provide personalized in-home therapy services to help patients regain independence, restore cognitive and physical function, and live their best lives all within the comfort of their homes.

Key Responsibilities:

• Conduct comprehensive in-home OT assessments and  develop individualized treatment plans
• Set short- and long-term goals to improve independence and daily function
• Educate and train patients and family members on therapeutic and self-care activities
• Instruct on the use of orthotic/prosthetic devices and custom adaptive equipment

• Perform ongoing functional and cognitive re-evaluations

• Collaborate with physicians, caregivers, and the broader home health team

• Maintain accurate clinical documentation and submit timely progress

  reports
• Support care coordination and refer patients for outpatient OT services when necessary
• Participate in team meetings, case conferences, and professional development

Qualifications (Required):

• Bachelors degree or higher in Occupational Therapy

• 1+ years of experience in Occupational Medicine

• 2+ years of experience in Social Work or relevant patient care settings

• Valid OTR license in the state of Pennsylvania (PA)

• Valid Drivers License

• One of the following valid certifications:

  • AHA BLS , AHA ACLS , or AHA PALS

• Must confirm:

  • You have not applied to or worked for Residential Home Health & Hospice within the last 6 months

  • You have spoken to a Relode recruiter about this role

Preferred:

• Please provide your OT license number upon application

Schedule:

• MondayFriday, 8am5pm

• No weekends, holidays, or on-call

Travel:

• Local travel required for in-home patient visits

• Mileage reimbursement provided
